Tips For: Recommendation Request
The goal in creating a ticket is to include enough information so that we can provide the help that you need in our first response. Otherwise, time and effort will be wasted on requests for more information and clarification. Please use the tips below to make the message body and the attachments to your email as effective as possible:

Please begin by identifying yourself. Include your current name and the name you were using when you took my course (if different). Identify the course number, course section, and semester in which you took the course. You do not need to provide extra details here. I will be asking you to fill out a form for each application that includes details (see below). Describe the application process that you would like me to support. To what are you applying? Will you be applying to more than one of them? If so, please provide a list. Include a description of the timeframe for the application(s). Include information regarding when a letter of recommendation would be due. |

The items listed below should be uploaded to the ticket created from this email. If you already have them available, then include them as attachments to this email. Otherwise, you can upload them later in a reply or attach them to a ticket that you have already created (see Replying and Opening the Ticket Page in Correspond).
